Responsibilities:
- Assist patients and residents with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting in LTC, rehab, or home health settings
- Monitor and accurately record vital signs such as blood pressure, pulse, temperature, and respiration rates, reporting changes to supervising nursing staff promptly
- Support safe patient mobility by assisting with transfers, ambulation, and repositioning using proper body mechanics and assistive devices
- Provide compassionate companionship and emotional support to patients and residents, fostering dignity and respect in every interaction
- Assist with feeding, hydration, and nutritional monitoring, documenting intake and output as directed by the care plan
- Maintain a clean, safe, and comfortable care environment by performing light housekeeping duties, changing linens, and ensuring infection control protocols are followed
- Communicate effectively with interdisciplinary care teams — including nurses, therapists, and social workers — to deliver coordinated, patient-centered care
- Observe and report changes in patient condition, behavior, skin integrity, or functional status to the charge nurse in a timely manner
- Follow all facility, agency, and Maryland state regulations regarding patient safety, HIPAA compliance, and documentation standards
- Participate in care plan meetings, staff training, and competency evaluations as required by the employer or Reliant Healthcare Solutions
Qualifications & Requirements for CNA Candidates in Baltimore, MD
- Active, unencumbered Certified Nursing Assistant (CNA) certification in the state of Maryland, in good standing with the Maryland Board of Nursing
- Successful completion of a state-approved CNA training program meeting Maryland COMAR requirements
- Minimum of a valid high school diploma or GED equivalent
- Current CPR/BLS certification from an AHA- or Red Cross-approved provider
- Ability to pass a Maryland state and federal criminal background check and drug screening as required by healthcare employers
- Demonstrated ability to lift, transfer, and reposition patients safely — typically up to 50 lbs — with or without reasonable accommodation
- Strong interpersonal and communication skills with the ability to work collaboratively within a multidisciplinary healthcare team
- Prior experience in a long-term care, rehabilitation, or home health setting is preferred but not required for entry-level candidates
