Responsibilities:

  • Assist patients and residents with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting in LTC, rehab, or home health settings
  • Monitor and accurately record vital signs such as blood pressure, pulse, respiration rate, and temperature, reporting changes to the supervising nurse promptly
  • Provide mobility assistance including transfers, repositioning, ambulation support, and proper use of assistive devices to prevent falls and pressure injuries
  • Deliver compassionate personal care to home health patients in their Durham-area residences, respecting patient dignity, privacy, and individual preferences
  • Support rehabilitation patients with prescribed exercise routines and therapy-directed activities under the guidance of licensed nursing and therapy staff
  • Maintain a clean, safe, and organized patient or resident environment in compliance with facility infection control and regulatory standards
  • Document care activities, patient observations, and behavioral changes accurately in electronic health records (EHR) or written care logs per facility protocol
  • Assist with meal preparation, feeding support, and hydration monitoring, ensuring dietary requirements and swallowing precautions are consistently followed
  • Communicate effectively with patients, families, and interdisciplinary healthcare team members to ensure coordinated, person-centered care delivery
  • Participate in shift handoffs, care plan meetings, and mandatory in-service training to maintain compliance and uphold quality standards across all healthcare settings

Qualifications & Requirements for CNA Candidates in Durham, NC

  • Active, unencumbered Certified Nursing Assistant (CNA) certification in the state of North Carolina, in good standing with the NC Nurse Aide I Registry
  • Minimum of a high school diploma or GED equivalent; completion of an accredited CNA training program required
  • Current CPR and Basic Life Support (BLS) certification from the American Heart Association or American Red Cross
  • Ability to pass a criminal background check and drug screening in accordance with North Carolina healthcare employment standards
  • Demonstrated ability to perform physical tasks including standing for extended periods, lifting up to 50 pounds, and assisting with patient transfers
  • Strong interpersonal and communication skills with the ability to interact compassionately with elderly, post-acute, and home-bound patient populations
  • Experience in at least one of the following settings preferred: long-term care, skilled nursing facility, inpatient rehabilitation, or home health
  • Reliable transportation and a valid driver's license required for home health positions serving Durham and surrounding Triangle-area communities
