Responsibilities:

• Assist residents and patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting in LTC, rehab, or home health settings
• Monitor and accurately record vital signs such as blood pressure, pulse, temperature, and respiration, reporting changes to the supervising nurse
• Support patient mobility by assisting with transfers, repositioning, and ambulation using proper body mechanics and assistive devices
• Deliver compassionate, person-centered care that respects the dignity, privacy, and cultural preferences of each individual patient or resident
• Assist with meal preparation, feeding, and hydration monitoring to ensure proper nutritional intake for assigned residents or home health clients
• Maintain a clean, safe, and organized care environment including making beds, sanitizing surfaces, and adhering to infection control protocols
• Document care activities accurately in electronic health records or paper charts in compliance with Pennsylvania Department of Health and facility standards
• Communicate effectively with interdisciplinary care teams including nurses, therapists, and social workers to support coordinated patient outcomes
• Observe and promptly report changes in a patient's physical or emotional condition, including signs of pain, distress, or altered mental status
• Provide emotional support and companionship to patients and residents, fostering trust and a sense of security in their care environment

Qualifications & Requirements for CNA Candidates in Philadelphia

• Active and unrestricted Pennsylvania CNA certification or eligibility to obtain PA certification prior to start date
• Current listing in the Pennsylvania Nurse Aide Registry with no substantiated findings of abuse, neglect, or misappropriation
• High school diploma or GED equivalent required
• Minimum of one (1) year of CNA experience preferred; new graduates with clinical training from accredited programs are encouraged to apply
• Current CPR/BLS certification from an AHA or Red Cross-approved provider
• Ability to pass a criminal background check, PA Child Abuse History clearance, and FBI fingerprint clearance as required by state law
• Physical ability to lift, transfer, and support patients, including standing for extended periods and performing repetitive physical tasks
• Strong interpersonal and communication skills with the ability to work collaboratively within a multidisciplinary healthcare team
