Responsibilities:
- Assist patients and residents with activities of daily living including bathing, dressing, grooming, and toileting in LTC, rehab, and home health settings
- Monitor and accurately record patient vital signs including blood pressure, temperature, pulse, and respiration rates, reporting changes to supervising nursing staff
- Support safe patient mobility by assisting with ambulation, transferring, and repositioning to prevent pressure injuries and promote rehabilitation goals
- Provide compassionate companionship and emotional support to residents and patients, particularly those in long-term care or receiving end-of-life services
- Assist with nutritional intake by preparing meal trays, feeding patients as needed, and documenting food and fluid consumption accurately
- Maintain a clean, safe, and dignified environment for patients by performing light housekeeping duties within the care space and following infection control protocols
- Communicate effectively with the interdisciplinary care team — including nurses, therapists, and physicians — to relay observations about patient condition and behavior
- Assist rehabilitation patients with prescribed therapeutic exercises under the guidance of physical or occupational therapists to support recovery milestones
- Document all care activities thoroughly in patient records in accordance with facility, state, and CMS compliance standards
- Follow all HIPAA regulations, facility safety policies, and Texas Department of Aging and Disability Services guidelines to ensure patient rights and confidentiality are protected
Qualifications & Requirements for CNA Candidates in San Antonio
- Active, unrestricted Certified Nursing Assistant (CNA) certification listed in good standing on the Texas Nurse Aide Registry (NAR)
- High school diploma or GED equivalent required
- Minimum of one completed clinical rotation or verifiable hands-on patient care experience preferred, though entry-level candidates with state certification are encouraged to apply
- Current CPR/BLS certification from an AHA or Red Cross accredited program
- Ability to pass a state and federal background check as required by Texas healthcare staffing regulations
- Demonstrated ability to lift, transfer, and reposition patients safely using proper body mechanics; must be able to lift up to 50 lbs with assistance
- Strong interpersonal communication skills and a genuine commitment to person-centered, compassionate care
- Availability for full-time hours with flexibility across day, evening, or night shifts depending on facility and patient needs
