Responsibilities:

  • Assist patients and residents with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting in LTC, rehab, and home health settings
  • Monitor and accurately record vital signs including blood pressure, pulse, respiration, and temperature, reporting any changes to the supervising nurse
  • Support patient mobility by assisting with ambulation, transfers, and proper repositioning to prevent pressure injuries and promote recovery
  • Provide compassionate companionship and emotional support to patients and their families, particularly in long-term care and home health environments
  • Maintain a clean, safe, and organized care environment in compliance with facility infection control protocols and Texas state health regulations
  • Assist with feeding and hydration, observing and documenting patients' nutritional intake and any swallowing concerns for the nursing team
  • Respond promptly to patient call lights and requests, prioritizing safety and dignity in every interaction
  • Communicate effectively with licensed nursing staff, therapists, and interdisciplinary team members to ensure continuity of patient-centered care
  • Accurately document care activities in electronic health records (EHR) or paper charting systems per facility and agency policy
  • Adhere to all HIPAA privacy standards, facility policies, and Texas Department of Aging and Disability Services (DADS) regulations governing CNA practice

What You'll Need to Qualify as a CNA in San Antonio

  • Active, unrestricted Certified Nursing Assistant (CNA) certification listed on the Texas Nurse Aide Registry (NAR) in good standing
  • High school diploma or GED equivalent required
  • Minimum completion of a state-approved CNA training program meeting Texas Health and Human Services Commission standards
  • Current CPR/BLS certification from the American Heart Association or American Red Cross
  • Prior experience in a long-term care, rehabilitation, or home health setting preferred but not required for entry-level candidates
  • Bilingual English/Spanish communication skills are a strong asset given San Antonio's predominantly bilingual patient population
  • Ability to pass a criminal background check and meet all pre-employment health screening requirements including TB test
