Responsibilities:

• Assist residents and patients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting in LTC, rehab, or home health settings
• Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration, reporting changes promptly to the supervising nurse
• Support safe patient mobility through transfers, repositioning, and ambulation using proper body mechanics and assistive devices
• Provide compassionate emotional support and meaningful interaction to patients and residents, fostering dignity and quality of life
• Maintain a clean, safe, and comfortable environment for patients, adhering to infection control protocols and facility-specific standards
• Assist rehabilitation patients with prescribed exercise routines and therapy-related activities under the direction of licensed clinical staff
• Accurately document care activities, observations, and patient responses in electronic health records (EHR) or paper charts per facility policy
• Communicate effectively with interdisciplinary care teams, families, and supervisors to ensure continuity and quality of patient care
• Respond promptly to call lights, patient requests, and emergency situations in accordance with facility protocols and safety standards
• Support home health patients with medication reminders, meal preparation, light housekeeping, and errands as outlined in the care plan

Qualifications We Look For in Wilmington CNA Candidates

• Active listing on the Delaware Nurse Aide Registry (DNAR) in good standing — required for all LTC and rehab placements
• Valid Certified Nursing Assistant (CNA) certification in the state of Delaware or eligibility for reciprocity from another state
• High school diploma or GED equivalent
• Minimum of 6 months of hands-on CNA experience preferred; new graduates with clinical training will be considered for select entry-level openings
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Demonstrated ability to assist with patient transfers and mobility using proper body mechanics; ability to lift up to 50 lbs as needed
• Strong interpersonal and communication skills with a patient-centered, empathetic approach to care
• Ability to pass a criminal background check, drug screening, and health clearance (including TB test) as required by Delaware state law and facility standards
