Home Health Physical Therapist Assistant (PTA) – PRN / Per Visit
Hyattsville, MD
Job Description
Reliant Healthcare Solutions (RHS) is seeking a skilled and self-motivated Home Health Physical Therapist Assistant (PTA) for PRN / Per Visit contract opportunities serving patients in the Washington, DC metro area. If you're a licensed PTA who values autonomy, meaningful one-on-one patient relationships, and the flexibility to build a schedule that works for your life, this is the opportunity you've been looking for. RHS is a trusted healthcare staffing partner with a reputation for connecting qualified clinicians to rewarding placements — and we're here to support you every step of the way.
In this home health PTA role, you won't be tied to a clinic caseload or a rigid nine-to-five. Instead, you'll choose the visits you accept, working as an extension of a supervising licensed Physical Therapist (PT) to deliver hands-on rehabilitative care directly in patients' homes across Washington, DC. You'll follow clearly developed plans of care, have access to a reachable supervising PT, and document your visits the same day using PostCareSoft — a streamlined mobile platform designed to keep your administrative burden low and your focus on patient outcomes. Whether you're an experienced home health PTA or a newer graduate ready to grow in a supported clinical environment, RHS wants to hear from you.
At RHS, we believe great patient care starts with empowered clinicians. That means giving you the tools, the supervision structure, and the scheduling freedom to do your best work — in the community, in patients' living rooms, and on your terms. Join a network of dedicated healthcare professionals making a measurable difference in the lives of Washington, DC residents every single day.

- Instruct patients and family caregivers in home exercise programs and safe movement techniques to promote independence and reduce rehospitalization risk
- Communicate changes in patient condition, functional status, or safety concerns to the supervising PT promptly and accurately
- Complete detailed, same-day visit notes in PostCareSoft via smartphone or tablet, ensuring documentation meets home health compliance and payer standards
- Adhere to Medicare home health conditions of participation and applicable DC and Maryland regulatory guidelines governing PTA scope of practice
- Collaborate with the interdisciplinary home health team — including RNs, occupational therapists, speech-language pathologists, and home health aides — to support holistic patient care
- Maintain professional communication with patients, caregivers, and the RHS clinical support team throughout each episode of care
- Please note: Evaluations, re-evaluations, and discharge visits are performed exclusively by the supervising licensed PT — consistent with industry standards and state practice regulations

- Minimum of 1 year of clinical experience as a licensed PTA in any setting; candidates with strong fieldwork and recent graduation will also be considered
- Demonstrated proficiency in therapeutic exercise, gait and balance training, transfer techniques, and fall prevention strategies
- Ability to work independently in a home health environment while maintaining consistent communication with a supervising PT
- Reliable personal vehicle, valid driver's license, and current auto insurance coverage — travel to patient homes throughout the Washington, DC metro area is required
- Comfortable using a smartphone or tablet for clinical documentation, time and attendance tracking, and care coordination via mobile apps such as PostCareSoft
