Home Health Physical Therapist (PT) – PRN / Per Visit
Clinton, MD
Job Description
Reliant Healthcare Solutions (RHS) is actively seeking a skilled and self-motivated Home Health Physical Therapist (PT) for PRN / Per Visit opportunities serving patients across Washington, DC, Montgomery County, MD, and Prince George's County, MD. This is your chance to practice on your terms — choosing the visits you want, building a schedule that fits your life, and delivering meaningful, one-on-one rehabilitative care right in the homes of patients who depend on your expertise. Whether you're looking to supplement your current caseload or build a flexible PT career without the constraints of a traditional clinic setting, RHS is the partner you've been waiting for.
At Reliant Healthcare Solutions, we believe great clinicians deserve great support. As a healthcare staffing agency with deep roots in the Mid-Atlantic region, we pride ourselves on connecting talented PTs with opportunities that match their clinical strengths and personal goals. From day one, our dedicated recruiter team works alongside you — handling the logistics so you can focus entirely on your patients. We offer competitive per-visit compensation, responsive communication, and a staffing experience built around your success, not a quota.
In this role, you'll carry out the full spectrum of home health PT services for a diverse patient population across the greater DC metro area. Using PostCareSoft on your phone or tablet, you'll complete timely, compliant visit documentation — evaluations, routine visits, re-evaluations, and discharge visits — keeping care coordination seamless and audit-ready. If you hold both an active Maryland PT license and a DC PT license, you'll have maximum flexibility to serve patients on both sides of the district line, expanding your per-visit earning potential significantly.
Key Responsibilities
- Conduct comprehensive initial evaluations and develop individualized physical therapy treatment plans for home health patients in DC, Montgomery County, and Prince George's County
- Perform skilled PT interventions targeting mobility, strength, balance, pain management, and functional independence in the home setting
- Complete routine visits, re-evaluations, and discharge visits in accordance with the plan of care and applicable Medicare/Medicaid home health guidelines
- Document all visit types accurately and promptly using PostCareSoft via mobile device, ensuring compliance with payer and regulatory requirements
- Communicate effectively with referring physicians, home health nurses, and interdisciplinary care team members to coordinate patient-centered outcomes
- Educate patients and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques
- Assess and recommend adaptive equipment or environmental modifications to promote patient safety and independence
- Adhere to HIPAA regulations and all applicable Maryland and DC state practice standards
- Maintain accurate mileage and visit logs to support billing and payroll processing
- Select visits and manage your own schedule with full autonomy, ensuring timely coverage of your accepted caseload
Requirements
- Active Maryland Physical Therapist (PT) license in good standing (required); active DC PT license strongly preferred for candidates serving Washington, DC patients
- Graduate of an accredited physical therapy program (DPT, MPT, or equivalent)
- Minimum of 1 year of clinical PT experience; prior home health PT experience highly preferred
- Current CPR/BLS certification (American Heart Association or equivalent)
- Demonstrated proficiency in conducting home health evaluations, skilled PT interventions, re-evaluations, and discharge assessments
- Reliable personal vehicle, valid driver's license, and current auto insurance meeting minimum state requirements for travel throughout DC, Montgomery County, and Prince George's County
- Ability to document visit notes efficiently using PostCareSoft or similar EMR/mobile documentation platforms
- Solid working knowledge of Medicare home health conditions of participation, therapy coverage criteria, and documentation standards
- Strong interpersonal and communication skills with the ability to engage patients, families, and interdisciplinary team members professionally
- Ability to work independently, manage a self-directed schedule, and maintain accountability without direct on-site supervision
Preferred: Experience with post-acute, geriatric, neurological, or orthopedic home health patient populations. Bilingual candidates (Spanish or Amharic) are encouraged to apply given the diverse communities served in the DC metro area.
Benefits
- Competitive per-visit compensation with separate evaluation rates and routine visit rates — maximize your earnings based on the visits you accept
- Total schedule autonomy — you select your visits and set your own hours with no mandatory minimums
- Weekly direct deposit pay so your earnings arrive on time, every time
- Dedicated RHS recruiter support from credentialing through ongoing assignment management
- Opportunity to serve patients across Washington, DC, Montgomery County, and Prince George's County, expanding your clinical footprint and per-visit volume
- Mobile-friendly documentation via PostCareSoft — spend less time at a desk and more time with patients
- Access to a network of home health partners across the broader Mid-Atlantic region for clinicians interested in expanding their PRN footprint
- Pathway to long-term or increased-volume engagements as RHS continues to grow its home health PT network in the greater DC metro area
