Flexible Part-Time Nursing Opportunities Across LTC, Home Health & Rehab Settings in NYC
Responsibilities:
- Administer medications and treatments as directed by the supervising RN or physician, accurately documenting all interventions in the patient health record
- Monitor and record vital signs, changes in patient condition, and responses to care plans in LTC, home health, or rehabilitation settings
- Provide skilled wound care, catheter care, and other clinical procedures in accordance with New York State nursing practice standards
- Assist residents and patients with activities of daily living (ADLs) including bathing, grooming, mobility support, and nutritional intake
- Collaborate with interdisciplinary care teams — including physical therapists, occupational therapists, social workers, and physicians — to implement and update individualized care plans
- Communicate effectively with patients, families, and care team members to ensure continuity of care and address concerns in a timely, compassionate manner
- Observe patients for signs of deterioration or adverse reactions and report findings promptly to supervising nursing staff or attending physicians
- Perform and document accurate intake and output measurements, glucose monitoring, and other routine clinical assessments as assigned
- Educate patients and family members on discharge instructions, medication management, and ongoing home health care protocols
- Maintain compliance with HIPAA regulations, facility infection control policies, and all applicable New York State Department of Health guidelines
Qualifications & Requirements for LPN/LVN Candidates in New York
- Active and unrestricted LPN or LVN license issued by the New York State Education Department (NYSED)
- Current Basic Life Support (BLS/CPR) certification from an AHA or Red Cross accredited program
- Experience in long-term care, home health, or rehabilitation nursing is preferred but candidates at all experience levels are encouraged to apply
- Demonstrated ability to administer medications, perform clinical assessments, and document care accurately using electronic health record (EHR) systems
- Strong interpersonal and communication skills with the ability to work effectively within diverse, multidisciplinary care teams
- Ability to meet the physical demands of the role, including standing for extended periods, assisting with patient transfers, and responding quickly in urgent situations
- Reliable transportation or ability to commute across NYC boroughs as required by home health or multi-site assignments
- Completion of any required facility-specific health screenings, immunizations, and background checks in accordance with New York State regulations
