Part-Time Licensed Practical & Vocational Nurse Opportunities Across Rochester's Top Care Settings

Responsibilities:

• Administer prescribed medications and treatments in accordance with New York State nursing practice standards and facility protocols
• Monitor and document patient vital signs, health status changes, and responses to care interventions in LTC, home health, or rehab environments
• Assist with the development and implementation of individualized patient care plans under the supervision of an RN or physician
• Provide wound care, catheter care, and other skilled nursing procedures as ordered in long-term care and rehabilitation settings
• Deliver compassionate personal care and support to patients in home health assignments, including bathing, mobility assistance, and medication management
• Collaborate with interdisciplinary care teams — including therapists, social workers, and physicians — to coordinate holistic patient outcomes
• Communicate effectively with patients, families, and caregivers to provide education on medications, care routines, and discharge instructions
• Maintain accurate and timely documentation in electronic health records (EHR) and paper-based systems as required by the facility
• Respond promptly to changes in patient condition and escalate concerns to supervising RNs or attending providers following established protocols
• Uphold infection control standards, HIPAA compliance, and facility-specific safety policies across all assigned care settings

Qualifications We're Looking For in Rochester LPN/LVN Candidates

• Current, active LPN or LVN license issued by the New York State Education Department (NYSED) in good standing
• Graduation from an accredited practical or vocational nursing program recognized by New York State
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Experience in long-term care, home health, or rehabilitation settings preferred but not required — new graduates are encouraged to apply
• Familiarity with electronic health record (EHR) systems such as PointClickCare, Meditech, or similar platforms is a plus
• Ability to work flexible shifts including days, evenings, nights, weekends, and rotating schedules as needed by assigned facilities
• Strong interpersonal and communication skills with the ability to work collaboratively in fast-paced, patient-centered environments
• Reliable transportation and ability to travel to various Rochester-area facilities or patient homes as required for home health assignments
