Home Care Marketing & Business Development Specialist

In this role, you will be the face of our home care division — cultivating referral relationships with discharge planners, social workers, physicians, skilled nursing facilities (SNFs), assisted living communities, and hospital case management teams. Your territory will span Silver Spring and the greater Montgomery County corridor, giving you the opportunity to make a tangible difference in how patients transition from acute care settings to home-based or facility-based care. Key Responsibilities

• Develop and execute a strategic business development plan to grow home health and nursing home referral volumes across Silver Spring, MD and surrounding communities
• Build and maintain relationships with hospital discharge planners, case managers, social workers (LCSW/MSW), physicians, and SNF administrators
• Conduct regular in-person and virtual outreach visits to referral sources, including assisted living facilities, skilled nursing facilities, and outpatient clinics
• Represent Reliant Healthcare Solutions at community health fairs, networking events, and professional association meetings throughout Montgomery County
• Collaborate with internal clinical and recruiting teams to ensure seamless patient intake and staffing fulfillment for referred home care cases
• Track, analyze, and report on referral trends, market penetration, and business development KPIs using CRM software
• Educate referral partners on RHS home health service offerings, including personal care, skilled nursing (RN/LPN), therapy (PT/OT/SLP), and companion care
• Develop and distribute marketing materials tailored to home care and post-acute care audiences
• Identify new market opportunities in underserved zip codes within the Silver Spring and greater DMV region
• Maintain compliance with all HIPAA regulations, state home health agency marketing guidelines, and CMS standards

• High school diploma or equivalent required; associate or bachelor's degree in healthcare administration, marketing, or business preferred
• Minimum of 2–4 years of business development, marketing, or sales experience within home health, home care, skilled nursing, or post-acute care settings
• Demonstrated success building referral relationships with hospital case managers, discharge planners, social workers (LCSW/MSW), and physician offices
• Strong working knowledge of home health regulations, CMS Conditions of Participation, and Maryland state home care licensing requirements
• Proficiency with CRM platforms (e.g., Salesforce, HubSpot, or similar) for tracking referral activity and pipeline development
• Excellent verbal and written communication skills with the ability to present confidently to clinical and administrative audiences
• Valid driver's license and reliable transportation for in-person outreach visits throughout the Silver Spring, MD and Montgomery County region
• Demonstrated ability to work independently in a remote environment while meeting or exceeding referral growth targets
• Familiarity with home health disciplines including skilled nursing (RN/LPN), physical therapy (PT), occupational therapy (OT), and personal care aide (PCA) service lines
• Strong organizational and time-management skills with the ability to manage multiple referral accounts simultaneously

Preferred: Prior experience working directly for a licensed home health agency or nursing home in a marketing or community liaison capacity. Bilingual candidates (Spanish/English or Amharic/English) are strongly encouraged to apply given the diverse patient population in the Silver Spring, MD corridor.

• Competitive annual salary range of $58,000–$65,000 based on experience, with performance-based incentive bonuses tied to referral volume growth
