Responsibilities:

• Provide direct, compassionate personal care to residents and patients in long-term care and rehabilitation settings, including bathing, grooming, dressing, and oral hygiene
• Assist patients with mobility, ambulation, transfers, and repositioning in accordance with individualized care plans and facility safety protocols
• Monitor and accurately document vital signs including blood pressure, pulse, respiration rate, temperature, and weight, reporting any abnormal findings to the supervising nurse
• Support residents during meal times by assisting with feeding, encouraging nutritional intake, and documenting food and fluid consumption as directed
• Respond promptly and professionally to patient call lights and requests, ensuring comfort, dignity, and a safe care environment at all times
• Assist the nursing team with restorative care activities and range-of-motion exercises tailored to rehabilitation patients working toward recovery goals
• Maintain a clean, safe, and organized patient environment including changing bed linens, sanitizing surfaces, and ensuring proper infection control practices are followed
• Observe and report changes in patient condition, behavior, or functional status to the charge nurse or clinical supervisor in a timely and accurate manner

• Listing in good standing on the Tennessee Nurse Aide Registry with no substantiated findings of abuse, neglect, or misappropriation
