Key Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and ambulation in home health and nursing home settings
- Monitor and accurately document vital signs — including blood pressure, pulse, respiration rate, and temperature — and promptly report changes in patient condition to supervising RNs or LPNs
- Support safe patient transfers and repositioning using proper body mechanics and approved assistive devices to prevent falls and pressure injuries
- Provide compassionate companionship and emotional support to patients and their families, fostering trust and dignity throughout the care continuum
- Assist with nutritional intake, including meal preparation, feeding support, and monitoring for signs of dysphagia or changes in appetite
- Maintain a clean, safe, and infection-controlled care environment in accordance with OSHA and facility-specific protocols
- Collaborate effectively within interdisciplinary care teams alongside RNs, LPNs, physical therapists, and social workers to implement individualized care plans
- Document care activities accurately in electronic health records (EHR) or patient care logs in compliance with HIPAA and state regulations
- Respond promptly and calmly to patient call lights, emergencies, or behavioral changes, demonstrating sound clinical judgment and composure
- Adhere to Virginia Board of Nursing standards and all applicable home health or skilled nursing facility regulatory requirements
Requirements
- Active Certified Nurse Assistant (CNA) certification in the state of Virginia, in good standing with the Virginia Board of Nursing
- Current listing on the Virginia Nurse Aide Registry with no substantiated findings of abuse, neglect, or misappropriation
- Valid CPR/BLS certification (American Heart Association or American Red Cross preferred)
- Demonstrated ability to perform ADL assistance, vital sign monitoring, and basic patient care procedures safely and competently
- Ability to meet the physical demands of the role including lifting up to 50 lbs, prolonged standing, and assisting with patient transfers
- Strong documentation skills with familiarity in electronic health record (EHR) systems or willingness to learn facility-specific platforms
- Knowledge of infection control practices, HIPAA compliance, and OSHA workplace safety standards
- Excellent interpersonal and communication skills with a patient-centered, empathetic approach to care delivery
- Reliable transportation and the ability to travel to patient homes or facilities within the Fredericksburg, VA metro area as required
Preferred: Prior experience in home health, home care, or long-term care/nursing home environments strongly preferred. Bilingual candidates (English/Spanish) are encouraged to apply.
