Responsibilities:

• Assist patients and residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and ambulation across LTC, rehab, home health, and clinic settings
• Monitor and accurately record vital signs such as blood pressure, pulse, respiration, temperature, and oxygen saturation, reporting changes in patient condition to supervising nursing staff
• Support safe patient transfers and repositioning using proper body mechanics and assistive equipment to prevent falls and pressure injuries
• Provide compassionate personal care to home health clients in Woodbridge-area residences, respecting patient dignity, privacy, and individual preferences at all times
• Assist with nutritional support by preparing meals, feeding patients as needed, and documenting food and fluid intake according to care plan requirements
• Maintain a clean, safe, and organized care environment by performing light housekeeping duties and ensuring proper infection control protocols are followed
• Communicate effectively with interdisciplinary care teams including nurses, physical therapists, and case managers to provide continuity of patient-centered care
• Document patient care activities, observations, and any changes in condition accurately in electronic health records (EHR) or facility-specific documentation systems

What We're Looking For in a Qualified CNA Candidate

• Active and unrestricted Certified Nursing Assistant (CNA) certification registered with the Virginia Board of Nursing (or eligibility to transfer certification from another state)
• High school diploma or GED equivalent required
• Current CPR and Basic Life Support (BLS) certification from an AHA or Red Cross accredited program
• Ability to pass a comprehensive background check, drug screening, and reference verification as required by Virginia state regulations and facility partners
• Strong interpersonal and communication skills with the ability to interact professionally with patients, families, and multidisciplinary healthcare team members
• Physical ability to lift, transfer, and reposition patients, stand for extended periods, and perform the physical demands associated with direct patient care
• Experience in one or more care settings — including long-term care, home health, rehabilitation, or outpatient clinic — is welcomed at all levels from new graduates to experienced CNAs
