Key Responsibilities

  • Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility support
  • Monitor and accurately document vital signs such as blood pressure, pulse, respiration rate, and temperature
  • Provide skilled assistance with transfers and ambulation using proper body mechanics and assistive devices
  • Support patients with meal preparation, feeding assistance, and hydration monitoring in accordance with care plans
  • Observe and report changes in patient condition, behavior, or functional status to the supervising RN or LPN promptly
  • Perform light housekeeping, laundry, and household tasks to maintain a safe and sanitary patient environment
  • Assist with catheter care, ostomy care, and skin integrity monitoring under nurse supervision
  • Maintain accurate, timely documentation of all patient care activities using electronic health record (EHR) systems
  • Collaborate with interdisciplinary care teams including RNs, LPNs, physical therapists, and social workers to ensure continuity of care
  • Adhere to all infection control protocols, HIPAA regulations, and agency-specific compliance standards while delivering patient-centered care in home and nursing home settings

Requirements

  • Active CNA certification in the state of Virginia (VA Board of Nursing registry in good standing)
  • Minimum high school diploma or GED required
  • Current CPR/BLS certification (American Heart Association or Red Cross accepted)
  • Experience with home health, personal care, or long-term care/nursing home settings welcomed at all levels — new graduates encouraged to apply
  • Demonstrated ability to assist with ADLs, vital sign monitoring, and patient transfers using safe handling techniques
  • Solid understanding of infection control practices, standard precautions, and OSHA compliance requirements
  • Ability to read, interpret, and follow individualized patient care plans developed by supervising clinical staff
  • Reliable transportation to reach patient homes throughout Manassas, VA and Prince William County service areas
  • Strong interpersonal and communication skills with the ability to build trust with patients, families, and clinical team members
  • Preferred: Prior experience using electronic health record (EHR) or EVV (Electronic Visit Verification) documentation systems
  • Preferred: Bilingual proficiency (English/Spanish) given the diverse patient population served in the Northern Virginia region
