Key Responsibilities

• Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and ambulation in home health and skilled care environments
• Monitor and accurately document vital signs including blood pressure, pulse, respiration rate, and temperature, reporting changes to the supervising RN or LPN
• Provide compassionate personal care and emotional support to patients recovering from illness, surgery, or managing chronic conditions
• Assist with safe patient transfers and repositioning using proper body mechanics and mechanical lift equipment to prevent falls and pressure ulcers
• Support nutritional needs by preparing and assisting with meal consumption, monitoring fluid intake, and reporting concerns related to appetite or dysphagia
• Maintain a clean, safe, and organized patient environment in compliance with infection control protocols and OSHA standards
• Collaborate with interdisciplinary care teams including RNs, LPNs, physical therapists, and case managers to deliver coordinated patient-centered care
• Observe and report changes in patient behavior, physical condition, or mental status that may indicate deterioration or the need for clinical reassessment
• Provide end-of-life comfort care and dignity-centered support to patients in hospice or palliative care settings when applicable
• Maintain accurate and timely documentation of care activities in accordance with facility and home health agency policies

• Active Certified Nurse Assistant (CNA) certification in the state of Virginia, in good standing with the Virginia Board of Nursing
• High school diploma or GED equivalent required
• Current CPR/BLS certification (American Heart Association or American Red Cross accepted)
• Experience with ADL assistance, vital signs monitoring, catheter care, and safe patient handling techniques
• Ability to work flexible shifts including days, evenings, nights, weekends, and rotating schedules in home health or nursing home environments
• Knowledge of and adherence to HIPAA privacy regulations, infection control standards, and state-specific CNA scope of practice guidelines
• Demonstrated ability to communicate clearly and professionally with patients, families, and multidisciplinary clinical teams
• Physical capability to lift up to 50 lbs, stand for extended periods, and perform hands-on patient care duties throughout an assigned shift
• Reliable transportation and ability to travel to patient homes within the Reston, VA and Northern Virginia service area (for home health assignments)
• Strong observational skills and commitment to documenting care accurately and in a timely manner

Preferred: Prior experience in home health, private duty care, or skilled nursing facility (SNF) settings. Bilingual candidates (Spanish/English or other languages) are strongly encouraged to apply.
