Responsibilities:

• Assist patients and residents with activities of daily living (ADLs) including bathing, grooming, dressing, and oral hygiene across LTC, home health, rehab, and clinic settings
• Monitor and accurately record vital signs such as blood pressure, pulse, temperature, and respiration rates, reporting changes to supervising nursing staff promptly
• Support patient mobility by assisting with transfers, repositioning, ambulation, and safe use of mobility aids to prevent falls and promote rehabilitation progress
• Provide compassionate, person-centered care to residents in long-term care facilities, respecting individual dignity, privacy, and cultural preferences at all times
• Assist home health patients with medication reminders, light housekeeping, meal preparation, and other supportive tasks as directed by the care plan
• Observe and document patient behavior, physical condition, and any changes in health status, maintaining accurate and timely healthcare records
• Collaborate effectively with licensed nursing staff, therapists, and interdisciplinary care teams to ensure seamless, high-quality patient care delivery
