Key Responsibilities

• Provide direct personal care assistance including bathing, grooming, dressing, and oral hygiene for home health and nursing home patients in McLean, VA
• Monitor and accurately record patients' vital signs including blood pressure, pulse, respiration, and temperature, reporting changes to supervising RNs or LPNs
• Assist patients with ambulation, transfers, and mobility support using proper body mechanics and patient-lift equipment as needed
• Support activities of daily living (ADLs) and instrumental activities of daily living (IADLs) to promote patient independence and dignity
• Deliver compassionate companionship and emotional support to elderly, chronically ill, or post-acute patients and their families
• Maintain a clean, safe, and organized patient environment in home care settings, including light housekeeping and meal preparation
• Assist with catheter care, wound observation, and skin integrity monitoring under the direction of a licensed nurse
• Communicate effectively with interdisciplinary care team members including RNs, LPNs, physical therapists, and case managers
• Document patient care activities and any observed changes in patient condition accurately in accordance with agency and regulatory requirements
• Adhere to all HIPAA privacy standards, infection control protocols, and state and federal home health compliance guidelines applicable to Virginia CNA practice

• Active Certified Nurse Assistant (CNA) certification in the Commonwealth of Virginia (or eligibility for reciprocity)
• High school diploma or GED required
• Current CPR/BLS certification (American Heart Association preferred)
• Ability to perform all ADL and IADL support tasks including transfers, ambulation assistance, and personal hygiene care
• Experience with or willingness to work in home care, home health, or skilled nursing facility (SNF) environments
• Strong knowledge of infection control practices, standard precautions, and OSHA compliance standards
• Reliable transportation and ability to travel to patient homes within the McLean, VA and broader Fairfax County area
• Demonstrated ability to communicate clearly and professionally with patients, family members, and clinical team members including RNs and LPNs
• Ability to work flexible shifts including days, evenings, nights, weekends, and rotating schedules based on patient and agency needs
• Commitment to maintaining patient confidentiality and adhering to HIPAA regulations at all times
• Preferred: Prior experience in geriatric care, dementia care, or working with medically complex home health patients
• Preferred: Bilingual communication skills (Spanish, Amharic, or other languages spoken in the Northern Virginia region)
