This role spans both home care and nursing home settings throughout Washington, DC, giving you the flexibility to work in environments that align with your experience and interests. You'll support patients with activities of daily living (ADLs), collaborate with licensed nursing staff, and serve as a critical touchpoint in each patient's care continuum. RHS placements at this level typically offer flexible scheduling including days, evenings, nights, weekends, and rotating shifts — designed to work around your life, not against it.
Key Responsibilities
- Assist patients with ADLs including bathing, grooming, dressing, toileting, and ambulation in both home health and skilled nursing facility environments
- Monitor and document vital signs — including blood pressure, pulse, respiration, and temperature — and promptly report changes in patient condition to supervising RNs or LPNs
- Provide compassionate companionship and emotional support to elderly and medically complex patients in Washington, DC home care settings
- Assist with safe patient transfers and repositioning using proper body mechanics and mechanical lift equipment to prevent pressure ulcers and fall-related injuries
- Support nutritional needs by preparing or serving meals, assisting with feeding, and monitoring fluid and dietary intake per care plans
- Maintain a clean, safe, and infection-controlled environment consistent with CMS guidelines and DC Department of Health regulations
- Communicate effectively with interdisciplinary care teams including RNs, LPNs, therapists, and case managers to ensure continuity of patient care
- Document care activities accurately in electronic health records (EHR) or paper charts as required by the facility
- Assist with range-of-motion exercises and restorative care activities as directed by licensed nursing or therapy staff
- Respond to patient call lights and urgent needs promptly, demonstrating professionalism and empathy in all interactions
Requirements
- Active CNA certification in Washington, DC (or certification eligible for DC reciprocity)
- High school diploma or GED required
- Current CPR/BLS certification (American Heart Association or Red Cross preferred)
- Experience in home care, home health, or skilled nursing facility (SNF) settings — all experience levels welcome to apply
- Demonstrated ability to safely perform patient transfers, repositioning, and mobility assistance using proper technique and mechanical lift equipment
- Working knowledge of infection control protocols, universal precautions, and HIPAA compliance standards
- Ability to accurately measure and document vital signs and report critical changes to supervising licensed nursing staff
- Reliable, punctual, and capable of working independently in home health environments with minimal direct supervision
- Strong interpersonal and communication skills with a patient-centered, empathy-driven care philosophy
- Preferred: Prior experience with memory care, dementia, or Alzheimer's patient populations
- Preferred: Familiarity with electronic health record (EHR) platforms such as MatrixCare, PointClickCare, or WellSky
