Responsibilities:

• Assist residents and home health clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor and accurately document vital signs, food and fluid intake, and changes in patient condition, reporting concerns promptly to supervising licensed nursing staff
• Provide compassionate companionship and emotional support to geriatric patients in long-term care and home health settings across the Reston, VA area
• Assist with safe patient transfers, repositioning, and ambulation using proper body mechanics and assistive devices to prevent falls and injury
• Support residents with feeding assistance and meal preparation in accordance with individualized care plans and dietary restrictions
• Maintain a clean, safe, and comfortable environment for patients, including changing bed linens, light housekeeping in home health settings, and infection control practices
• Collaborate effectively with interdisciplinary care teams, including RNs, LPNs, physical therapists, and social workers, to deliver coordinated patient-centered care
• Adhere strictly to HIPAA regulations and facility or agency privacy policies when handling confidential patient information
• Observe and report behavioral, cognitive, or physical changes in geriatric patients that may indicate emerging health concerns or changes in mental status
• Participate in required staff meetings, in-service training, and continuing education as requested by facility management or home health agency supervisors

Qualifications & Requirements for GNA Candidates in Reston, VA

• Current, valid Geriatric Nursing Assistant (GNA) certification issued or recognized by the state of Virginia (VDOE/VBOT registry in good standing)
• Active listing on the Virginia Nurse Aide Registry with no substantiated findings of abuse, neglect, or misappropriation
• High school diploma or GED equivalent preferred; candidates currently enrolled in nursing programs are encouraged to apply
• Minimum of one (1) year of experience in a long-term care, skilled nursing, or home health setting preferred; new GNA graduates with clinical training are welcome
• Current CPR/BLS certification from an AHA or Red Cross-approved provider
• Demonstrated ability to communicate clearly and empathetically with geriatric patients, family members, and multidisciplinary healthcare teams
• Ability to meet the physical demands of the role, including lifting, bending, standing for extended periods, and assisting with patient transfers
• Reliable transportation and the ability to travel to patient homes or facilities within the Reston, VA and broader Fairfax County service area as required for home health assignments
