Key Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and ambulation in home health and nursing home settings
- Monitor and accurately document vital signs such as blood pressure, pulse, respiration rate, and temperature, reporting changes to the supervising RN or LPN
- Provide compassionate companionship and emotional support to patients and their families, particularly those with chronic illness, cognitive decline, or post-acute care needs
- Assist with safe patient transfers and repositioning using proper body mechanics and assistive devices to prevent falls and pressure injuries
- Support patients with meal preparation, feeding assistance, and nutritional monitoring in compliance with individualized care plans
- Maintain a clean, safe, and therapeutic home or facility environment, including light housekeeping tasks relevant to patient care areas
- Communicate clearly and professionally with interdisciplinary care teams, including RNs, LPNs, therapists, and case managers
- Follow all infection control protocols, HIPAA regulations, and state and federal home health care compliance guidelines
- Document care activities accurately in electronic health records (EHR) or written logs as required by the facility or home health agency
- Respond promptly and calmly to patient needs and emergency situations, following established protocols and escalating to clinical supervisors as appropriate
Requirements
- Active CNA certification in the Commonwealth of Virginia (or eligibility to transfer certification)
- High school diploma or GED required
- Current CPR/BLS certification (American Heart Association or Red Cross accepted)
- Experience in home care, home health, or skilled nursing facility (SNF) settings preferred; entry-level candidates with clinical externship or training experience welcome
- Demonstrated ability to perform ADL assistance, vital signs monitoring, and basic patient care procedures with accuracy and compassion
- Solid understanding of infection control, universal precautions, and OSHA safety standards as they apply to home health environments
- Ability to follow individualized patient care plans developed by RNs or supervising clinicians with minimal supervision
- Reliable transportation to travel to patient homes or facility locations across Herndon, VA and surrounding Northern Virginia communities
- Strong interpersonal and communication skills with the ability to build trust with patients, families, and multidisciplinary team members
- Ability to pass a pre-employment background check, drug screening, and TB test in accordance with Virginia home health regulations
- Preferred: Prior experience using electronic health record (EHR) platforms such as HHAeXchange, ClearCare, or similar home health software
- Preferred: Bilingual candidates (Spanish or other languages spoken in the Herndon/Northern Virginia area) are strongly encouraged to apply
