Key Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility support in both home care and nursing home settings
- Monitor and accurately document patient vital signs including blood pressure, pulse, respiration rate, and temperature, reporting changes to supervising RN or LPN promptly
- Provide compassionate companionship and emotional support to patients and their families, particularly for elderly or chronically ill individuals in Woodbridge-area homes
- Assist with safe patient transfers and ambulation using proper body mechanics and assistive devices to prevent injury
- Support patients with meal preparation, feeding assistance, and nutritional intake monitoring in accordance with individualized care plans
- Maintain a clean, safe, and comfortable environment for patients, including light housekeeping duties relevant to the patient's care setting
- Follow infection control protocols, including proper hand hygiene, PPE usage, and standard precautions as required by Virginia state and facility-specific guidelines
- Communicate effectively with interdisciplinary care teams, including RNs, LPNs, physical therapists, and social workers, to ensure continuity of care
- Document care activities accurately in EMR or paper-based systems in compliance with HIPAA and facility policies
- Respect and uphold each patient's dignity, privacy, and individual care preferences in every interaction
Requirements
- Active and unrestricted CNA certification in the state of Virginia (or eligibility to transfer certification)
- Minimum of a high school diploma or GED equivalent
- Current listing in good standing on the Virginia Nurse Aide Registry
- Experience in home care, home health, or nursing home/long-term care settings preferred; entry-level candidates with clinical externship experience are encouraged to apply
- Demonstrated ability to perform ADL assistance, vital signs monitoring, and safe patient handling techniques
- Familiarity with infection control procedures, standard precautions, and OSHA workplace safety guidelines
- Ability to lift and transfer patients safely, with the physical stamina to perform duties across extended shifts including nights, evenings, and weekends
- Strong interpersonal and communication skills with the ability to work collaboratively within a multidisciplinary healthcare team
- Reliable transportation to serve clients and facilities across the Woodbridge, VA area, including Prince William County locations
- Must pass a background check, drug screening, and provide verifiable professional or clinical references
Preferred: Prior experience with electronic medical records (EMR) systems such as WellSky, Homecare Homebase, or PointClickCare. Bilingual candidates (English/Spanish) are strongly encouraged to apply given the diverse patient population in the Woodbridge, VA region.
