Responsibilities:

• Assist patients and residents with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility support across LTC, home health, rehab, and clinic environments
• Monitor and accurately record vital signs including blood pressure, temperature, pulse, and respiration rates, reporting changes in patient condition to supervising nursing staff promptly
• Provide compassionate companionship and emotional support to patients, particularly those in long-term care or home health settings who may experience social isolation or cognitive decline
• Assist with patient transfers and repositioning using proper body mechanics and mechanical lift equipment to ensure safety for both the patient and care team
• Support rehabilitation patients with prescribed therapeutic exercises and mobility activities under the direction of licensed nursing and therapy staff
• Maintain a clean, safe, and organized patient care environment by changing linens, sanitizing equipment, and adhering to infection control and OSHA protocols
• Document patient care activities accurately and timely in electronic health records (EHR) or paper-based systems in compliance with facility and regulatory standards
• Assist clinical staff during procedures, examinations, and treatments within the CNA scope of practice as directed by the charge nurse or supervising clinician
