Temporary RN Opportunities in Home Health, Rehab & Long-Term Care Across Prince William County

Responsibilities:

• Conduct comprehensive patient assessments across home health, rehabilitation, and long-term care settings to establish individualized plans of care
• Administer prescribed medications, IV therapies, and wound care treatments in accordance with physician orders and facility or agency protocols
• Monitor and document patient progress, vital signs, and clinical status using electronic health record (EHR) systems, communicating changes promptly to the interdisciplinary care team
• Develop, implement, and evaluate patient-centered care plans in collaboration with physicians, therapists, social workers, and family caregivers
• Provide skilled nursing visits in patients' homes, ensuring a safe environment and educating patients and caregivers on disease management and self-care techniques
• Facilitate smooth care transitions between acute, rehabilitation, and home health settings to minimize readmissions and promote continuity of care
• Educate patients, families, and caregivers on medication regimens, fall prevention, chronic disease management, and post-procedural recovery expectations
• Coordinate with home health aides, physical therapists, occupational therapists, and speech-language pathologists to deliver holistic, team-based care
• Maintain accurate, timely clinical documentation that meets Medicare, Medicaid, and Joint Commission compliance standards
• Respond to patient and family concerns with empathy and professionalism, serving as a primary clinical point of contact throughout the care episode

Qualifications We're Looking For in Woodbridge-Area RNs

• Current, active Registered Nurse (RN) licensure in the Commonwealth of Virginia or a valid multistate Nurse Licensure Compact (NLC) license in good standing
• Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program — new graduates and experienced RNs welcome
• Current BLS/CPR certification through the American Heart Association or American Red Cross; ACLS preferred for certain assignments
• Experience in at least one of the following settings is preferred but not required: home health, skilled nursing facility (SNF), long-term care, or inpatient rehabilitation
• Familiarity with electronic health record platforms such as Kinnser, PointClickCare, MatrixCare, or similar home health and LTC documentation systems
• Strong clinical assessment, critical thinking, and time management skills with the ability to prioritize independently in community-based or facility settings
• Valid driver's license and reliable personal transportation for home health visit assignments throughout the Woodbridge and Prince William County area
• Ability to pass a comprehensive background check, drug screening, and health clearance (including TB test and immunization verification) as required by client facilities
