Temporary RN Opportunities in Home Health, Rehab & Long-Term Care Across Fairfax, VA

Responsibilities:

• Conduct thorough patient assessments and develop individualized care plans in home health, rehabilitation, or long-term care settings across Fairfax County
• Administer medications, IV therapies, wound care, and other prescribed treatments in accordance with physician orders and state of Virginia nursing practice standards
• Monitor and document changes in patient condition, vital signs, and response to treatment using facility or agency electronic health record (EHR) systems
• Collaborate with interdisciplinary care teams including physicians, therapists, social workers, and home health aides to ensure coordinated, patient-centered care
• Educate patients and family members on disease management, medication regimens, post-acute recovery protocols, and community health resources available in Fairfax, VA
