Responsibilities:

• Assist residents and patients with activities of daily living including bathing, grooming, dressing, and oral hygiene in LTC and rehab settings
• Monitor and accurately document vital signs including blood pressure, temperature, pulse, and respiration rates and report changes to the nursing team
• Provide mobility assistance including repositioning bedridden residents, assisting with transfers using proper body mechanics and lift equipment
• Support post-acute rehabilitation patients with ambulation exercises and encourage active participation in prescribed therapy routines
• Respond promptly to call lights and patient requests to ensure comfort, dignity, and timely care delivery
• Assist with nutritional needs by serving meals, providing feeding support when required, and monitoring dietary intake
• Maintain a clean, safe, and organized patient environment in compliance with facility infection control and safety protocols
• Communicate effectively with licensed nursing staff, therapists, and interdisciplinary care team members regarding changes in resident condition or behavior
