• Assist residents and patients with activities of daily living (ADLs) including bathing, dressing, grooming, and oral hygiene in LTC and rehab settings
• Monitor and accurately document vital signs, intake and output, and any observable changes in patient condition, reporting findings promptly to the supervising nurse
• Provide mobility assistance and safe patient transfers using proper body mechanics and assistive devices, supporting rehabilitation goals and fall prevention protocols
• Deliver compassionate, person-centered care to long-term care residents, respecting dignity, privacy, and individual preferences at all times
• Assist with feeding, hydration, and nutritional support for residents who require help during mealtimes, including those with swallowing precautions
• Respond promptly to call lights and patient requests, maintaining a calm and reassuring presence for residents experiencing physical or emotional distress
• Collaborate with the interdisciplinary care team — including nurses, physical therapists, and occupational therapists — to support individualized rehabilitation and care plans
• Maintain a clean, safe, and organized resident environment by adhering to infection control standards, facility housekeeping protocols, and OSHA guidelines
