Part-Time Licensed Practical & Vocational Nurse Opportunities Across LTC, Home Health & Rehab Settings in Gilbert, AZ

Responsibilities:

• Administer prescribed medications and treatments in accordance with Arizona State Board of Nursing regulations and facility protocols
• Monitor and document patient vital signs, health status changes, and care outcomes in electronic health record (EHR) systems
• Provide direct patient care in long-term care, home health, or rehabilitation settings including wound care, catheter care, and post-surgical support
• Collaborate with registered nurses, physicians, therapists, and interdisciplinary care teams to develop and execute individualized patient care plans
• Conduct thorough patient and family education on medication regimens, discharge instructions, and ongoing health management strategies
• Observe and report changes in patient condition, behavior, or functional status to the supervising RN or attending physician promptly
• Assist with activities of daily living (ADLs) and support patient dignity, comfort, and independence across all care settings
• Maintain accurate, timely, and compliant nursing documentation in accordance with CMS, state, and facility-specific standards
• Perform skilled nursing procedures including IV therapy (where permitted by scope), tube feedings, ostomy care, and respiratory treatments
• Participate in care conferences, quality improvement initiatives, and ongoing in-service training to maintain clinical competency

Qualifications & Licensing Requirements for LPN/LVN Candidates in Gilbert, AZ

• Active and unrestricted LPN or LVN license issued by the Arizona State Board of Nursing (AZBN) or a valid Nurse Licensure Compact (NLC) license
• Graduation from an accredited practical or vocational nursing program recognized by the AZBN
