Part-Time Licensed Practical & Vocational Nursing Opportunities Across Long-Term Care, Home Health & Rehabilitation Settings

Responsibilities:

• Administer prescribed medications and treatments in accordance with Arizona State Board of Nursing standards and facility protocols
• Monitor and document patient vital signs, health status changes, and responses to care plans in long-term care, home health, or rehabilitation settings
• Collaborate with registered nurses, physicians, and interdisciplinary care teams to implement and update individualized patient care plans
• Provide direct patient care including wound care, catheter care, IV management (where permitted by scope), and personal hygiene assistance
• Educate patients and family members on care procedures, discharge instructions, medication regimens, and wellness strategies
• Observe and report significant changes in patient condition promptly to the supervising RN or attending physician
• Maintain accurate and timely clinical documentation in electronic health record (EHR) systems in compliance with facility and regulatory requirements
• Assist with patient intake, assessments, and care coordination across home health visits or rehabilitation therapy sessions
• Adhere to infection control protocols, HIPAA regulations, and all applicable state and federal healthcare compliance standards
• Support a dignified, patient-centered environment by actively contributing to a culture of safety, empathy, and clinical excellence

Qualifications & Requirements for LPN/LVN Candidates in Tucson
