Part-Time Licensed Practical & Vocational Nurse Opportunities Across Phoenix's Growing Healthcare Landscape

Responsibilities:

  • Administer prescribed medications and treatments in accordance with Arizona State Board of Nursing guidelines and facility protocols
  • Monitor and document patient vital signs, health status changes, and responses to care plans in long-term care, home health, and rehabilitation settings
  • Assist in developing, implementing, and updating individualized patient care plans under the supervision of a Registered Nurse or physician
  • Provide wound care, catheter care, IV maintenance, and other skilled nursing interventions as permitted within Arizona LPN/LVN scope of practice
  • Communicate effectively with patients, families, interdisciplinary care teams, and home health aides to ensure continuity and quality of care
  • Educate patients and caregivers on medication regimens, disease management, discharge instructions, and wellness strategies
  • Accurately complete clinical documentation using electronic health record (EHR) systems, ensuring compliance with facility and regulatory standards
  • Support rehabilitation patients with therapeutic activities, mobility assistance, and progress monitoring in coordination with PT/OT staff
  • Conduct home health assessments and safety evaluations for patients in Phoenix-area residences, identifying and escalating concerns promptly
  • Maintain infection control standards, HIPAA compliance, and patient dignity across all care environments

Qualifications & Licensing Requirements for LPN/LVN Candidates in Arizona

  • Active, unrestricted LPN or LVN license issued by the Arizona State Board of Nursing (or a Nurse Licensure Compact license valid in AZ)
  • Current BLS/CPR certification from the American Heart Association or American Red Cross
  • Minimum of one completed clinical rotation or prior work experience in LTC, home health, or rehabilitation — all experience levels considered
  • Demonstrated ability to perform skilled nursing procedures within the Arizona LPN/LVN scope of practice
  • Familiarity with electronic health record (EHR) platforms such as PointClickCare, MatrixCare, or similar systems preferred
  • Strong interpersonal and communication skills with the ability to work collaboratively within interdisciplinary healthcare teams
  • Reliable transportation and valid driver's license required for home health assignments in the greater Phoenix metro area
  • Ability to pass a pre-employment background check, drug screening, and health clearance in compliance with Arizona healthcare facility requirements
