Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehabilitation Settings Across Mesa, AZ
Responsibilities:
- Administer medications and treatments in accordance with physician orders and Arizona state nursing practice standards across LTC, home health, and rehabilitation settings
- Monitor and document patient vital signs, health status changes, and responses to care interventions in accurate and timely medical records
- Provide skilled wound care, catheter maintenance, IV therapy support, and other clinical procedures as permitted under LPN/LVN scope of practice in AZ
- Collaborate with RNs, physicians, therapists, and interdisciplinary care teams to implement and update individualized patient care plans
- Conduct patient and family education on medication management, disease processes, home care techniques, and discharge planning instructions
- Perform thorough patient assessments during home health visits and communicate findings to supervising RNs and care coordinators promptly
- Support activities of daily living and restorative nursing care for long-term care residents, promoting dignity, independence, and quality of life
- Assist rehabilitation patients with therapeutic exercises and mobility support as directed by physical and occupational therapists
- Maintain compliance with HIPAA regulations, infection control protocols, and facility or agency-specific policies and procedures
- Accurately complete all required nursing documentation including care notes, incident reports, and medication administration records in a timely manner
Qualifications We're Looking For in Mesa LPN/LVN Candidates
- Active, unencumbered Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license issued by the Arizona State Board of Nursing (AZBN)
- Current Basic Life Support (BLS/CPR) certification through the American Heart Association or American Red Cross
- Completion of an accredited practical or vocational nursing program from an accredited institution
- Experience in long-term care, home health, or rehabilitation nursing preferred but new graduates with clinical training in these settings are welcome to apply
- Demonstrated ability to provide compassionate, patient-centered care to elderly, post-acute, and medically complex populations
- Familiarity with electronic health record (EHR) systems and proficiency in accurate clinical documentation practices
- Reliable transportation and valid Arizona driver's license required for home health assignments within the Mesa and greater Maricopa County area
- Strong communication skills and ability to collaborate effectively within interdisciplinary healthcare teams across varied care environments
