Part-Time Licensed Practical & Vocational Nurse Opportunities in LTC, Home Health & Rehabilitation Settings Across Greater Los Angeles

Responsibilities:

  • Administer prescribed medications and treatments in compliance with California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) standards and facility protocols
  • Monitor and document patient vital signs, symptoms, and changes in condition across LTC, home health, or rehabilitation settings, escalating concerns to the supervising RN or physician as appropriate
  • Provide skilled wound care, dressing changes, catheter care, and IV therapy management per care plan orders
  • Assist patients with activities of daily living (ADLs) and support functional rehabilitation goals in coordination with physical, occupational, and speech therapy teams
  • Conduct thorough, accurate nursing assessments and contribute to individualized patient care plan development and updates
  • Educate patients and family members regarding medication regimens, disease management, home care instructions, and safety precautions
  • Maintain detailed, timely clinical documentation in electronic health records (EHR) systems in accordance with facility and regulatory requirements
  • Collaborate effectively with interdisciplinary care teams including RNs, physicians, therapists, social workers, and home health aides to ensure continuity and quality of care
  • Observe and enforce infection control protocols and maintain a safe, clean care environment for all patients
  • Participate in patient transitions and discharge planning processes, providing skilled nursing support during care transitions within LTC, home health, or rehab settings

Qualifications & Requirements for LPN/LVN Candidates in Los Angeles

  • Active and unrestricted Licensed Vocational Nurse (LVN) license issued by the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) — required
  • Candidates holding an LPN license from another U.S. state with eligibility for California endorsement are encouraged to apply
  • Current BLS/CPR certification from the American Heart Association or American Red Cross
  • Experience in one or more of the following settings preferred but not required: long-term care (LTC), home health, or inpatient/outpatient rehabilitation
  • Familiarity with California-specific nursing regulations, HIPAA compliance, and state-mandated documentation standards
  • Demonstrated ability to work independently and exercise sound clinical judgment, particularly in home health environments with limited direct supervision
  • Proficiency with electronic health record (EHR) platforms; experience with PointClickCare, Homecare Homebase, or similar systems is a plus
  • Strong interpersonal and communication skills with the ability to build trust with patients, families, and multidisciplinary team members from diverse cultural and linguistic backgrounds reflective of the Los Angeles community
