Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehabilitation Across Fresno, California

Responsibilities:

• Administer medications and treatments as prescribed by the supervising physician or RN in LTC, home health, or rehab settings
• Monitor and document patient vital signs, health status changes, and responses to care interventions
• Assist with developing and implementing individualized care plans under the direction of a Registered Nurse or care team
• Provide wound care, catheter care, and other skilled nursing procedures consistent with the care setting and scope of practice
• Communicate patient condition updates and significant changes to supervising RNs, physicians, and interdisciplinary team members
• Support patients with activities of daily living (ADLs) and promote functional independence in rehabilitation environments
• Conduct home health visits to Fresno-area patients, performing assessments and delivering hands-on nursing care in the home setting
• Educate patients and family members on medication management, disease processes, and post-care instructions
• Maintain accurate and timely clinical documentation in compliance with California state regulations and facility or agency standards
• Collaborate with physical therapists, occupational therapists, social workers, and other care team members to support holistic patient outcomes

Qualifications & Requirements for LPN/LVN Candidates in Fresno, CA
