Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Across Silicon Valley
Responsibilities:
- Administer prescribed medications and treatments to patients in LTC, home health, or rehabilitation settings in accordance with California LVN scope of practice
- Monitor and document patient vital signs, intake/output, and changes in condition, reporting significant findings to the supervising RN or physician promptly
- Perform wound care, dressing changes, catheter care, and other skilled nursing procedures as directed by the care plan
- Assist with the development and implementation of individualized patient care plans in collaboration with the interdisciplinary healthcare team
- Provide compassionate personal care and support activities of daily living (ADLs) for patients in long-term care and home health environments
- Educate patients and family members on medication regimens, discharge instructions, and disease management techniques relevant to their care plan
- Accurately maintain electronic health records (EHR) and ensure all clinical documentation meets facility, state, and regulatory standards
- Support rehabilitation patients with therapeutic exercises, mobility assistance, and restorative care activities under the direction of the rehab team
- Communicate effectively with patients, families, and interdisciplinary staff to ensure continuity and quality of care across all settings
- Adhere to infection control protocols, HIPAA regulations, and all applicable California Department of Public Health (CDPH) standards
What We're Looking For: LPN/LVN Qualifications & Requirements
- Current, active Licensed Vocational Nurse (LVN) license issued by the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) — no restrictions
- Current BLS/CPR certification from the American Heart Association or American Red Cross (required prior to placement)
- Minimum of one completed clinical rotation or any level of professional experience in LTC, home health, or rehabilitation settings preferred but not required for all roles
- Demonstrated ability to administer medications safely, including subcutaneous injections and IV therapy where applicable and within California LVN scope
- Strong clinical assessment and critical thinking skills with the ability to recognize and escalate changes in patient condition appropriately
- Proficiency with electronic health record (EHR) systems; experience with PointClickCare, MatrixCare, or similar platforms a plus
- Reliable transportation and the ability to travel to patient homes or multiple facility locations within the San Jose metropolitan area for applicable roles
- Excellent interpersonal and communication skills with sensitivity to serving San Jose's diverse, multilingual patient population
