Responsibilities:

• Assist residents and patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and ambulation in LTC and rehabilitation settings
• Monitor and accurately document vital signs including blood pressure, temperature, pulse, and respiration, reporting any changes to the supervising nurse promptly
• Support rehabilitation patients with prescribed exercise routines and mobility assistance under the direction of physical and occupational therapy staff
• Provide compassionate emotional support and companionship to long-term care residents, promoting dignity and quality of life throughout daily interactions
• Assist with safe patient transfers and repositioning using proper body mechanics and facility-approved lifting equipment to prevent injury to patients and staff
• Maintain a clean, safe, and organized patient environment, including making beds, sanitizing surfaces, and ensuring rooms comply with California state infection control standards
• Respond promptly to call lights and patient requests, triaging needs appropriately and escalating urgent concerns to licensed nursing staff
