Part-Time Licensed Nursing Opportunities Across Long-Term Care, Home Health & Rehabilitation Settings

Responsibilities:

• Administer prescribed medications and treatments in accordance with California BRN regulations and facility protocols across LTC, home health, and rehabilitation settings
• Monitor and document patient vital signs, health status changes, and responses to care plans, escalating concerns promptly to supervising RNs or physicians
• Assist in developing and implementing individualized care plans tailored to the needs of long-term care residents, home health clients, or rehabilitation patients
• Perform wound care assessments and dressing changes, including post-surgical and chronic wound management as directed by the care team
• Coordinate with multidisciplinary teams — including physical therapists, occupational therapists, social workers, and physicians — to ensure seamless, patient-centered care delivery
• Educate patients and family members on medication regimens, disease management strategies, home safety practices, and post-discharge care instructions
• Maintain accurate, timely electronic health records (EHR) and nursing notes in compliance with California state regulations and facility documentation standards
• Assist with activities of daily living (ADLs) and ensure patient comfort, dignity, and safety are upheld in all care environments
• Conduct intake assessments and contribute to discharge planning processes, particularly for rehabilitation patients transitioning back to home or community-based care
• Follow all infection control, HIPAA compliance, and facility-specific policies to maintain a safe and therapeutic environment for patients and staff

Qualifications We're Looking For in San Diego LPN/LVN Candidates

• Current, active LPN or LVN license issued by the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT) — no compact license accepted in lieu of CA licensure
• Graduation from an accredited Licensed Practical or Vocational Nursing program recognized by the state of California
• Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
• Experience in at least one of the following settings is preferred but not required: long-term care/skilled nursing facility, home health, or inpatient/outpatient rehabilitation
• Familiarity with electronic health record (EHR) systems such as PointClickCare, Epic, or Cerner is a plus
• Strong knowledge of California Title 22 regulations and nurse-to-patient staffing requirements applicable to LTC and skilled nursing settings
• Excellent interpersonal communication skills with the ability to effectively engage patients, families, and interdisciplinary healthcare teams
• Ability to work flexible part-time shifts, including days, evenings, or weekends, based on facility and patient needs across San Diego County
