Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Across Chandler, Arizona

Responsibilities:

• Administer prescribed medications and treatments in accordance with Arizona Board of Nursing scope of practice guidelines and facility protocols
• Monitor, document, and report changes in patient condition to supervising registered nurses and attending physicians across LTC, home health, and rehab settings
• Perform wound care, catheter care, and other hands-on clinical procedures as directed by the care plan
• Conduct routine vital sign assessments and accurately record findings in electronic health record (EHR) systems used by Chandler-area facilities
• Assist with the development, implementation, and updating of individualized patient care plans in collaboration with the interdisciplinary care team
• Provide patient and family education regarding medications, discharge instructions, and ongoing home health care needs
• Support rehabilitation patients with therapeutic activities and ADL assistance under the direction of physical, occupational, or speech therapy staff
• Maintain accurate and timely nursing documentation in compliance with CMS, state, and facility-specific regulatory requirements
• Communicate effectively with patients, families, and healthcare team members to ensure continuity of care across all assigned shifts
• Uphold infection control standards and safety protocols to ensure a safe care environment for patients and staff in all healthcare settings
