Responsibilities:

• Assist residents and patients with activities of daily living (ADLs) including bathing, grooming, dressing, and oral hygiene in LTC and rehab environments
• Monitor and accurately document vital signs such as blood pressure, pulse, respiration rate, and temperature, reporting changes to the charge nurse promptly
• Support mobility and safe ambulation of rehabilitation patients, including proper use of assistive devices such as walkers, gait belts, and wheelchairs
• Provide compassionate, person-centered care to long-term care residents, maintaining dignity and respect in every interaction
• Assist licensed nursing staff with wound care observations, catheter care, and other delegated clinical tasks per Wisconsin state guidelines
• Respond promptly to call lights and patient requests, ensuring timely and attentive care throughout each shift
• Assist with feeding, hydration, and meal setup for residents or patients with varying levels of dietary and physical needs
