Responsibilities:

• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting in LTC and rehab settings
• Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration rates, reporting changes to the supervising nurse
• Support safe patient mobility and ambulation, including use of gait belts, hoyer lifts, and assistive devices per care plans
• Provide compassionate emotional support and companionship to long-term care residents and rehabilitation patients throughout recovery
• Assist with meal preparation, feeding, and hydration monitoring in accordance with each resident's individualized nutrition plan
• Perform routine skin assessments and report any signs of pressure injuries, redness, or skin breakdown to the charge nurse
• Maintain a clean, safe, and dignified living environment for residents by performing light housekeeping and linen changes
• Participate in interdisciplinary care team meetings and contribute observations about resident progress, behavior changes, or concerns
• Accurately complete nursing assistant documentation and electronic health record (EHR) entries in compliance with Ohio state regulations
• Adhere to all infection control protocols, HIPAA guidelines, and facility-specific policies to ensure resident and staff safety

Qualifications & Requirements for CNA Candidates in Toledo, OH

• Active and unrestricted Ohio CNA certification in good standing with the Ohio Nurse Aide Registry
• High school diploma or GED equivalent required
• Minimum completion of a state-approved CNA training program; experience in LTC or rehab settings preferred but not required
• Current CPR and Basic Life Support (BLS) certification or willingness to obtain prior to assignment start
