Responsibilities:

• Assist residents and patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting in accordance with individualized care plans
• Monitor and record vital signs such as blood pressure, pulse, respiration rate, and temperature, promptly reporting any abnormal findings to the supervising nurse
• Provide mobility assistance including transferring patients using proper body mechanics, gait belts, and mechanical lift equipment to prevent injury
• Support rehabilitation goals by encouraging and assisting patients with prescribed exercise routines and ambulation under the direction of therapy and nursing staff
• Maintain a safe, clean, and comfortable environment for all residents, including routine room upkeep and proper infection control practices
• Assist with nutritional intake by serving meals, feeding patients who require assistance, and documenting food and fluid intake accurately
• Respond promptly and compassionately to call lights and patient requests, ensuring dignity and respect are upheld in every interaction
• Document care delivery accurately in electronic health records (EHR) or paper charts, adhering to facility-specific protocols and Ohio state regulations
• Communicate effectively with interdisciplinary care teams including nurses, therapists, and social workers to ensure continuity of quality resident care
• Observe and report changes in patient behavior, skin integrity, mood, or physical condition to the charge nurse to support timely clinical interventions

Qualifications & Requirements for CNA Candidates in Columbus, OH

• Current, active Certified Nursing Assistant (CNA) certification listed in good standing on the Ohio Nurse Aide Registry (ONAR)
• Valid government-issued photo identification and Social Security card or equivalent employment eligibility documentation
• High school diploma or GED equivalent required
