Part-Time LPN & LVN Opportunities in Long-Term Care, Home Health & Rehabilitation Settings Across Columbus, OH

Responsibilities:

  • Administer prescribed medications and treatments to patients in LTC, home health, or rehabilitation settings in accordance with Ohio state nursing regulations
  • Monitor and document patient vital signs, health status changes, and responses to treatment, reporting critical findings to supervising RNs or physicians promptly
  • Provide direct, compassionate personal care including wound dressing changes, catheter care, and assistance with activities of daily living (ADLs)
  • Collaborate with interdisciplinary care teams — including physical therapists, occupational therapists, and social workers — to support individualized patient care plans
  • Educate patients and their families on medication regimens, discharge instructions, and chronic disease self-management strategies
  • Accurately complete and maintain patient medical records and nursing documentation in compliance with facility, state, and federal guidelines
  • Conduct patient intake assessments and assist in developing and updating individualized care plans under the direction of a Registered Nurse
  • Support rehabilitation patients in completing prescribed therapy exercises and mobility activities, promoting recovery and independence
  • Maintain a safe, clean, and infection-controlled care environment in alignment with OSHA standards and facility infection prevention protocols
  • Respond effectively to patient emergencies, initiating basic life support measures and coordinating with emergency personnel as needed

Qualifications We're Looking For in Columbus LPN/LVN Candidates

  • Active and unrestricted LPN or LVN license issued by the Ohio Board of Nursing (or a compact state license recognized in Ohio)
  • Completion of an accredited Practical Nursing or Vocational Nursing program from an approved educational institution
  • Current CPR/BLS certification from the American Heart Association or American Red Cross
  • Experience in one or more of the following settings is a plus but not required: long-term care, skilled nursing facilities, home health, or inpatient rehabilitation
  • Strong clinical assessment skills with the ability to recognize and respond to changes in patient condition in a timely manner
  • Proficiency in electronic health records (EHR) documentation; familiarity with systems such as PointClickCare or MatrixCare is advantageous
  • Excellent interpersonal and communication skills with a patient-centered, empathetic approach to care delivery
  • Ability to work independently with minimal supervision, particularly in home health environments requiring self-direction and professional judgment
