Part-Time LPN & LVN Opportunities in Long-Term Care, Home Health & Rehab Across the Indianapolis Metro Area
Responsibilities:
- Administer prescribed medications and treatments in accordance with Indiana state nursing standards and facility protocols
- Monitor and document patient vital signs, health status changes, and responses to care plans across LTC, home health, and rehab settings
- Collaborate with registered nurses, physicians, and interdisciplinary care teams to deliver coordinated, patient-centered care
- Assist in developing, implementing, and updating individualized care plans for residents and home health patients
- Perform wound care, catheter care, IV therapy (as permitted by scope of practice), and other skilled nursing procedures
- Provide compassionate personal care support and education to patients and family members regarding treatment plans and self-care techniques
- Accurately maintain and update electronic health records (EHR) and nursing documentation in compliance with facility and regulatory requirements
- Observe and report changes in patient condition to supervising RNs or attending physicians in a timely and professional manner
- Ensure adherence to infection control protocols, HIPAA regulations, and all applicable federal and Indiana state healthcare guidelines
- Participate in care conferences, shift handoffs, and ongoing staff education initiatives as requested by facility leadership
Qualifications & Licensing Requirements for LPN/LVN Candidates in Indiana
- Current, active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in good standing with the Indiana Professional Licensing Agency (IPLA)
- Graduation from an accredited LPN/LVN nursing program approved by the Indiana State Board of Nursing
- Current CPR/BLS certification from the American Heart Association or American Red Cross
- Experience in one or more of the following settings preferred: long-term care, skilled nursing facility, home health, or inpatient/outpatient rehabilitation
- Familiarity with electronic health record (EHR) systems such as PointClickCare, Epic, or MatrixCare is a plus
- Ability to pass a comprehensive background check, drug screen, and health screening per facility and state requirements
- Strong clinical assessment, critical thinking, and patient communication skills appropriate to the LPN/LVN scope of practice
- Reliable transportation and the ability to travel to patient homes or multiple facility locations within the Indianapolis metro area as required
