Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Across Greater Cincinnati

Responsibilities:

  • Administer prescribed medications and treatments in accordance with Ohio Board of Nursing standards and facility protocols across LTC, home health, and rehab settings
  • Monitor and document patient vital signs, health status changes, and clinical progress, reporting significant findings promptly to supervising RNs or physicians
  • Assist with wound care, catheter care, ostomy management, and other skilled nursing procedures as directed by the care plan
  • Collaborate with interdisciplinary care teams — including occupational therapists, physical therapists, and social workers — to support patient rehabilitation and recovery goals
  • Provide direct patient care in home health environments, supporting activities of daily living, medication management, and family caregiver education
  • Maintain accurate and timely electronic health records (EHR) documentation in compliance with facility, state, and federal regulatory requirements
  • Conduct patient and family education on discharge planning, chronic disease management, and post-procedure home care instructions
  • Observe patients for adverse reactions to medications or treatments and initiate appropriate interventions within LPN/LVN scope of practice
  • Participate in care conferences, shift handoffs, and quality improvement initiatives to promote continuity and excellence in patient care
  • Ensure a safe, respectful, and dignified care environment for residents, patients, and clients across all assigned healthcare settings

Qualifications We're Looking For in Cincinnati-Area LPN/LVNs

  • Active, unrestricted LPN or LVN license issued by the Ohio Board of Nursing (or current multistate Nurse Licensure Compact eligibility)
  • Graduation from an accredited practical or vocational nursing program approved by an applicable state board of nursing
  • Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
  • Ability to work part-time with flexible shift availability including days, evenings, nights, or weekends depending on facility needs
  • Experience in long-term care, home health, or rehabilitation settings is preferred but not required — new graduates with clinical rotations in these settings are encouraged to apply
  • Proficiency with electronic health record (EHR) systems and standard clinical documentation practices
  • Strong interpersonal, communication, and critical thinking skills with a demonstrated commitment to patient-centered, compassionate care
  • Ability to pass a background check, drug screening, and provide required immunization or health records in compliance with Ohio state and facility requirements
