Part-Time Licensed Practical & Vocational Nurse Opportunities Across Chicago's Leading Care Settings

Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring in LTC, home health, and rehabilitation settings throughout Chicago and surrounding communities
• Administer prescribed medications and treatments in accordance with Illinois nursing practice standards and facility-specific protocols
• Observe, record, and report changes in patient condition to supervising registered nurses or physicians to ensure timely and appropriate interventions
• Assist patients with activities of daily living (ADLs) including bathing, grooming, mobility support, and nutritional intake in home health and long-term care environments
• Collaborate with interdisciplinary care teams — including physical therapists, occupational therapists, and social workers — to support individualized patient rehabilitation plans
• Maintain accurate and thorough clinical documentation using electronic health record (EHR) systems in compliance with facility and Illinois state regulatory requirements
• Educate patients and family members on care plans, medication schedules, disease management strategies, and post-discharge instructions
• Perform catheter care, tube feedings, IV therapy monitoring, and other skilled nursing procedures as permitted within Illinois LPN scope of practice
• Ensure a safe, clean, and therapeutic environment for patients and residents while adhering to infection control and OSHA standards
• Participate in care planning meetings, shift huddles, and ongoing staff education initiatives to promote continuous quality improvement across assigned healthcare settings

Qualifications & Licensing Requirements for Chicago LPN/LVN Candidates

• Active, unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license issued by the Illinois Department of Professional Regulation (IDFPR)
• Graduation from an accredited practical or vocational nursing program recognized by the Illinois Board of Nursing
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Experience in one or more of the following care settings is a plus but not required: long-term care (LTC), home health, or inpatient/outpatient rehabilitation
