Part-Time Licensed Practical & Vocational Nurse Opportunities in LTC, Home Health & Rehab Settings Across Toledo

Responsibilities:

  • Administer prescribed medications and treatments in accordance with Ohio Nurse Practice Act guidelines and facility protocols across LTC, home health, and rehab settings
  • Monitor and document patient vital signs, health status changes, and responses to care plans, reporting significant findings promptly to supervising RNs or physicians
  • Assist in developing, implementing, and updating individualized patient care plans in collaboration with interdisciplinary care teams
  • Provide direct hands-on patient care including wound care, catheter management, ostomy care, and post-surgical rehabilitation support
  • Conduct patient and family education on medication management, disease processes, discharge planning, and home care techniques
  • Maintain accurate and timely clinical documentation in electronic health record (EHR) systems in compliance with CMS and Ohio Department of Health standards
  • Coordinate with home health aides, therapists, and social workers to ensure continuity and quality of care across all service touchpoints
  • Observe and report changes in patients' physical, mental, and emotional conditions to support proactive, patient-centered interventions
  • Assist with admissions, transfers, and discharge procedures while ensuring patient dignity and regulatory compliance throughout each transition
  • Uphold infection control standards, HIPAA compliance, and safety protocols specific to long-term care, rehabilitative, and home-based care environments

Qualifications & Requirements for LPN/LVN Candidates in Toledo, OH

  • Active, unrestricted LPN or LVN license issued by the Ohio Board of Nursing (OBN) or a qualifying compact state license recognized in Ohio
  • Graduation from an accredited practical or vocational nursing program approved by a state board of nursing
  • Current BLS/CPR certification from the American Heart Association or American Red Cross
  • Ability to work flexible part-time shifts, including days, evenings, or weekends, based on facility and patient needs across Toledo-area care sites
