Part-Time LPN & LVN Opportunities Across Long-Term Care, Home Health, and Rehab Settings in the Pittsburgh Area
Responsibilities:
- Administer prescribed medications and treatments in accordance with Pennsylvania state nursing practice standards and facility protocols
- Monitor and document patient vital signs, functional status, and clinical changes across LTC, home health, and rehabilitation settings
- Assist in the development, implementation, and updating of individualized patient care plans in collaboration with RNs and interdisciplinary team members
- Provide direct bedside care including wound dressing changes, catheter care, feeding assistance, and personal hygiene support
- Communicate effectively with patients, families, and supervisory nursing staff regarding changes in patient condition or care needs
- Conduct home health visits throughout assigned Pittsburgh-area territories, performing skilled nursing assessments and patient education
- Support rehabilitation patients in meeting functional goals by reinforcing therapy routines and tracking progress under RN or physician direction
- Maintain accurate, timely documentation in electronic health records (EHR) systems in compliance with facility and regulatory requirements
- Adhere to infection control protocols, HIPAA regulations, and Pennsylvania Department of Health standards at all times
- Participate in care conferences, shift handoffs, and ongoing staff education initiatives as required by the employing facility or agency
Qualifications & Requirements for LPN/LVN Candidates in Pittsburgh, PA
- Active, unrestricted LPN or LVN license issued by the Pennsylvania State Board of Nursing (or compact state eligibility where applicable)
- Graduation from an accredited practical or vocational nursing program recognized by the Pennsylvania State Board of Nursing
- Current CPR/BLS certification from the American Heart Association or American Red Cross
- Ability to pass a Pennsylvania-compliant background check including federal and state criminal history and PA Child Abuse History clearance
- Experience in one or more of the following settings is preferred but not required: long-term care, home health, or inpatient/outpatient rehabilitation
- Proficiency in electronic health record (EHR) documentation; experience with PointClickCare, MatrixCare, or similar platforms is a plus
- Strong clinical assessment and critical thinking skills with the ability to recognize and escalate changes in patient condition appropriately
- Reliable transportation and valid driver's license required for home health assignments within the Greater Pittsburgh area
