Flexible Part-Time Nursing Opportunities Across Long-Term Care, Home Health, Rehab & Clinic Settings

Responsibilities:

• Administer prescribed medications and treatments in accordance with Maryland Board of Nursing standards and facility protocols across LTC, home health, rehab, and clinic settings
• Monitor and document patient vital signs, health status changes, and responses to treatment, promptly reporting significant findings to the supervising RN or physician
• Assist in developing, implementing, and updating individualized patient care plans in collaboration with the interdisciplinary care team
• Provide direct patient care including wound care, catheter management, ostomy care, and IV therapy maintenance as permitted under Maryland LPN/LVN scope of practice
• Conduct thorough patient and family education on medication regimens, disease management, discharge instructions, and preventive health practices
• Support activities of daily living (ADLs) for patients in long-term care and home health settings, ensuring dignity, comfort, and safety at all times
• Maintain accurate and timely electronic health records (EHR) and nursing documentation in compliance with HIPAA regulations and facility-specific standards
• Coordinate with home health aides, therapy staff, and care coordinators to ensure seamless continuity of care for patients across community and facility-based settings
• Perform admissions assessments, routine health screenings, and follow-up evaluations in clinic environments under the direction of a supervising provider
• Respond appropriately to patient emergencies, initiate basic life support measures when necessary, and communicate effectively with emergency response teams

Qualifications We're Looking For in Our Gaithersburg LPN/LVN Candidates

• Current, active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensure issued by the Maryland Board of Nursing (MBON) or a valid Compact Nurse License recognized in Maryland
• Graduation from an accredited practical or vocational nursing program approved by a recognized state board of nursing
• Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
• Ability to work flexible part-time shifts including days, evenings, nights, weekends, and rotating schedules across one or more healthcare settings
• Proficiency with electronic health record (EHR) systems; experience with PointClickCare, MatrixCare, or similar platforms preferred for LTC and home health settings
• Strong interpersonal and communication skills with the ability to collaborate effectively within multidisciplinary healthcare teams and engage compassionately with patients and families
• Experience in long-term care, home health, outpatient rehabilitation, or clinical settings is welcomed at all levels — new graduates and seasoned LPN/LVNs are encouraged to apply
• Reliable transportation and the ability to travel to patient homes or multiple facility locations within the Gaithersburg and greater Montgomery County area, as applicable to assignment type
