Temporary RN Opportunities in Home Health, Rehab & Long-Term Care Across Gaithersburg and Montgomery County

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans in home health, rehab, or long-term care settings
• Administer medications, IV therapies, wound care, and other prescribed treatments in accordance with Maryland Board of Nursing standards and facility protocols
• Collaborate with interdisciplinary care teams including physicians, physical therapists, social workers, and case managers to ensure coordinated patient outcomes
• Monitor and document patient progress, vital signs, and responses to treatment in electronic health record (EHR) systems
• Provide patient and family education on disease management, medication adherence, and post-discharge care instructions
• Coordinate discharge planning and transition-of-care services to ensure continuity and reduce preventable readmissions
• Respond promptly to changes in patient condition and initiate appropriate clinical interventions or escalate to supervising physicians as needed
