Temporary RN Opportunities in Home Health, Rehab & Long-Term Care Across Rockville, Maryland

Responsibilities:

• Conduct comprehensive patient assessments in home health, rehabilitation, or long-term care settings and develop individualized plans of care in collaboration with interdisciplinary teams
• Administer prescribed medications, IV therapies, and wound care treatments in accordance with physician orders and facility or agency protocols
• Monitor and document patient progress, vital signs, and clinical outcomes using electronic health record (EHR) systems, ensuring accuracy and regulatory compliance
• Educate patients and family members on disease management, medication regimens, post-procedural care, and safe home environment practices
• Coordinate care transitions between hospital, rehabilitation, home health, and long-term care settings to promote continuity and reduce readmission risk
• Collaborate with physical therapists, occupational therapists, social workers, and physicians to support holistic patient recovery and functional independence
• Recognize and respond to changes in patient condition, initiate emergency protocols when necessary, and communicate critical updates to the supervising provider in a timely manner
• Maintain compliance with Maryland Board of Nursing standards, HIPAA regulations, and facility-specific policies and procedures at all times
