Responsibilities:
- Administer prescribed medications and treatments in accordance with Maryland Board of Nursing standards and facility protocols across LTC, home health, rehab, and clinic settings
- Monitor and document patient vital signs, health status changes, and responses to treatment, reporting significant findings to supervising RNs or physicians promptly
- Provide direct, hands-on patient care including wound care, catheter management, IV maintenance (where permitted by scope), and personal hygiene assistance in long-term care and rehabilitation environments
- Conduct structured patient assessments at the start of each shift and update individualized care plans in collaboration with the interdisciplinary care team
- Educate patients and family members on medication regimens, discharge instructions, home care techniques, and chronic disease management strategies
- Perform clinical intake procedures in outpatient clinic settings including vital signs, health history documentation, and preparation of patients for provider examinations
- Maintain accurate and timely electronic health record (EHR) documentation in compliance with facility, state, and federal regulatory requirements
- Support rehabilitation therapy teams by reinforcing exercise programs, mobility assistance, and restorative nursing activities for post-acute and long-term care patients
- Coordinate with home health care teams to ensure continuity of care, schedule follow-up visits, and communicate patient condition updates to case managers and supervisors
- Uphold infection control standards, safety protocols, and HIPAA compliance in all care settings to protect patient and staff well-being
Qualifications & Requirements for LPN/LVN Candidates in Bowie, MD
- Active, unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license issued or recognized by the Maryland Board of Nursing
